Welcome to Transition House

As Northumberland County’s only homeless shelter, we provide safe, dignified and supportive temporary housing, life skills and transitional support services to men and women over the age of 18 from Cobourg, Port Hope, Grafton, Colborne, Campbellford, Brighton, Hamilton Township, and Alderville. Our service area includes all of Northumberland County.

Transition House strives to be a client-focused, supportive, collaborative and accountable organization. At the core of our strategic directions is our commitment to clients, staff, partners, volunteers, donors, funders and the community. In these challenging times of COVID 19 Transition House is following all public health protocols and ensuring the use of masks and social distancing while on site. Currently, we are operating at a reduced capacity however work closely with Northumberland County to ensure housing based service access 24/7. We remain open for support and inter-agency referral for those looking for connections. Since the beginning of the pandemic we’ve not missed one opportunity for service and will remain operational for all county residents needing this assistance.
