Transition House Emergency Shelter has been providing safe and supportive shelter for Northumberland County’s most compromised citizens since 1999. As the only emergency shelter that serves all citizens of Northumberland County, Transition House is committed to offering a valuable and often life-saving service for those with multiple challenges, particular vulnerabilities and needs related to homelessness and housing insecurity.

Transition House utilizes a Housing First philosophy, providing not only emergency shelter, but assistance in rapid rehousing by mobilizing a variety of community partners and supports. With an integrated and individualized approach residents at Transition House take part in developing a personal action plan and staff work with them to access the appropriate community resources and services that are available in the community. Transition House has strong community partnerships and is an access point to a boarder system of supports and services.

We recognize that secure and lasting housing is fundamental to human dignity and is a basic human rights issue. Transition House supports a Housing First approach that recognizes the need for a co-ordinated systems response that focuses on re-housing people as quickly as possible and helps residents access the supports they need to stabilize their lives.

Transition House strives to create a supportive atmosphere where staff and residents work together to promote a safe, secure environment that provides stability and structure to the residents’ lives.

Residential Services

Transition House is a 4 bedroom, 18 bed shelter with dormitory accommodation that assists men and women over the age of 18. We have supportive staff on site 24 hours a day 7 days a week to assist those living within the shelter or for individuals who require our services.

Our Programs
Our Shelter provides a variety of services to our residents including:

  • Safe and secure temporary accommodations for up to 90 days
  • 24 hour support for residents
  • Goal setting and action planning
  • Assistance finding permanent housing
  • Financial management assistance
  • Weekly house meetings
  • Weekly paramedicine visits
  • Employment assistance
  • Referring residents to agencies within the community
  • Facilitating and advocating on behalf of residents with community agencies and resources
